If the ship you are on is sunk during a PvP battle between two flags at war or while taking part in a sinking blockade, you have a very slim chance of getting an injury. The injury will either be in the form of an eyepatch in place of an eye, a pegleg in place of a leg, or a hook in place of a hand. You can only have one of each injury on a pirate, for a maximum of three injuries per pirate.

For PvP injuries, you must belong to the flag whose ship is sunk to have a chance of an injury.

For the price of 100,000 PoE, you can have an injury removed by a Ringer.
