The Ye known world item is a map, located in the chart section of the booty panel, which shows all of your memorized league points and all of the islands that you have set foot upon. You may view it at any time by clicking upon it and selecting "View" from the resulting menu.

The Ye known world map did not always exist. During that time, pirates were forced to hop aboard a ship to see their map of the ocean.
